The Health and Social Care (the Alliance) Scotland is National third sector intermediary for a range of Health and Social care organisations. The Community Link Practioners work in communities to link people of all ages with disability, long term conditions and unpaid carers with services to have their voices heard, to enjoy the right to live well, as equal active citizens, free from discrimination, with support services that place them at the centre.
